Thota V Reddy lives in India. He was 59 when he was diagnosed in January, 2010. His initial PSA was 8.10 ng/ml, his Gleason Score was 6, and he was staged T4. His initial treatment choice was Surgery (Orchiectomy) and his current treatment choice is ADT-Androgen Deprivation (Hormone) (ADT3). Here is his story.

Hi, I am T. V. Reddy (59), from Hyderabad, India -diagnosed with metastatic advanced -prostate cancer, spread to pelvic lymph nodes and left acetabulum with PSA 8,and Gleason Score 3+3=6. I have undergone orchiectomy ten days back in Hyderabad. No other health problems till now.

I underwent a TURP (Trans Urethral Resection of the Prostate) for BPH (Benign Prostate Hyperplasia) a year back in October 2008 - sadly biopsy sample was missed by hospital authorities, and urologist failed to estimate the disease in very frequent visits in DRE (Digital Rectal Examination). No new biopsy and PSA were recommended as doctor ruled out cancer - suspected BPH, but, now the disease was confirmed as metastatic advanced prostate cancer, spread to bone.

PSA level was 8.1
Prostate size is 53x50x42mm (estimated volume 60cc) heterogenous echotexture, median lobe extending into bladder.
TRUS guided prostate biopsy positive for malignancy- Gleason Score 6(3+3)- 6/6 linear cores are infiltrated tumour - focal perineural spread seen in 1/6 cores from the right side.
MRI scan-enlarged prostate with heterogenous signal intensity, focal breach of prostate capsule, involvement of urinary bladder trigone, left obturator lymphadenopathy, left hip joint (acetabulum, proximal femur) lesions - rule out prostatic malignancy with secondaries.
Bone scan - left acetabulum finding suspicious for metastasis
Rest no other health problems noted and no body and bone pains

Subcapsular orchidectomy done on 8 January 2010.
Medication - Idrofos 50mg, Biprosta (Casodex) 50mg, Vitamin B complex and Vitamin D pills-follow up after 2 months.

Bilateral orchiectomy was done immediately in January 2010 and I was put on medication of Casodex 50mg + Idrofos 50mg + Vitamin D 50mg 4. as I was also diagnosed with cholesterol at the same time. I am taking Lipicure-10mg for that, and Vitamin B and Vitamin C tablets.

My PSA numbers have been:

January 2010 - 8.10
March 2010 - 0.90
July 2010 - 0.20
September 2010 - 0.70
October 2010 - 0.90
November 2010 - 1.70 (after stopping Casodex)
December 2010 - 1.0
February 2011 - 2.0 (again Casodex switched on)
March 2011 - 1.3

As I developed a small reddish patch on my penis (past 8 months) I was referred to a dermatologist, biopsy was done to rule out Erythroplasia of Queyrat [Bowen's Disease]. It was diagnosed as general infection I am on Candid (ointment) for that.

Right from beginning I hadn't developed any problems, symptoms or side effects - especially no urinary problems. Until now no weakness. Daily I am going for 10 km walk (as I have for the past 25 years) and eat a vegetarian diet, taking plenty of fruits and vegetables.

I am unable to understand why my PSA is climbing silently. Although I have written to many urologists and oncologists and urologists like Dr Snuffy Myers I haven't got a reply from anyone.

April 2012

- PSA slowly starting to rise, but within normal values.--6.26ng/ml (0.2 to 6.7ng/ml)

- Testosterone 27ng/dl--normal(247-827ng/dl)

- On juicing (tomato + carrot + pomegranate) multivitamin pills and good diet.

- On 2nd line harmone therapy(honvan).,past 8 months.

- Now and then antibiotics for urine infections.

- Now on (29-3-2012), prostate size increased to (42*51*48mm), projection in to bladder, volume 55cc. mild to moderate hydro ureteronephrosis on right side./creatinine and kidney sizes normal.

- Acid,alkaline phosphatases normal.

- Bone scan shows increased tracer concentration at various bones.,no clarity of bone degeneration or met's?

- Lucky to have good urologist--dr shyam.r.varma (hyderabad-india).

- Planning to rush to chemo--docetaxel+predinsone.,stopping honvan

- Urologist is planning for dj/nephrostomy.,after 2 to 3 chemo cycles.

- Planned to change idrofos-50 to 150mg

- Other tablets used are (roliten-od+uripass+tamdura+alfad3)

Hi, I am TS Reddy, son of Thota V Reddy, writing on behalf of my dad.

Diagnosed with locally advanced prostate cancer on 2010 January (Gleason score=6(3+3) and PSA; 8.1 ng/ml), left acetablem suspicious for bone scan, urinary bladder trigone and pelvic lymph node involved, 6/6 linear cores infiltrated tumour.

Orchiectomy done on 8th Jan 2013, kept on casodex-50mg, idrofos-50+multivitamins.

After being on casodox-50mg for 1.8 years (2010 Jan to 2011 May), and on honvan 200mg, twice a day for few months (2011 June to 2012 April), PSA; risen to 6.0ng/ml - then gone for chemo - docetaxel + predinsone, 6 cycles (April 2012 to August 2012) - though PSA; gone to 2 ng/ml. (lot of dietary turmeric+garlic+ginger was used during chemotherapy). In August 2012, PSA; came back again within two months to 12.72ng/ml.

Gone for imrt (radiation) to bone and prostate in November 2012, after that again kept on casodox-50mg+multi vitamins including saw palmetto and lycopene.

PSA gone down to 6.0ng/ml in Feb 2013. It raised again to 23 ng/ml on June 2013.

Switched off and on the same casodox-50mg, mean while tried some alternative medicine (soursoup leaves) - but resulted in urinary inconsistence. But PSA; started rising, reached all-time high, right from diagnosis to 50ng/ml. as on 10th August 2013 and also new lesion came up in sacrum, resulting in severe pain, unable to walk freely, and severe urinary inconsistence too.

Again honvan 200mg/twice a day. Started along with idrofos-50mg, lycopene, 50mg, multi vitamins, fish oil supplements, vitamin-d and vitamin c, noscopine 10ml/day, and roliten-od (200mg).

Planning for ketoconazole (after seeking advice from survivours and doctors, especially from (joel nowak.,of malecare advanced prost ca programme) who had suggested keto. a year back, itself.
