Don Rose and Karen live in California, USA. He was 66 when he was diagnosed in July, 2013. His initial PSA was 8.20 ng/ml, his Gleason Score was 6, and he was staged T1c. His initial treatment choice was Non-Invasive (Active Surveillance) and his current treatment choice is None. Here is his story.

THERE WAS NO RESPONSE TO AN UPDATE REMINDER IN 2015 SO THERE IS NO UPDATE.

In my last 3 years my PSA went from 4.8 down to 3.7 and up to a 8.2 this year. My GP sent me to a urologist who found nothing unusual with a digital exam. He scheduled me for an ultrasound and biopsy.

The results ~~~~>

~ preoperative diagnosis = elevated PSA

~ 2 slides with Adenocarcinoma
- 0.1 cm in rt lateral base involving 10% of tissue
- 0.3 cm in rt lateral mid involving 30% of tissue

~ a high grade prostatic intraepitheleal neoplasia (high grade PIN) in the left lateral mid

~ no perineural invasion by the tumoor was detected

Urology Doc said that it fell into the range of intermediate risk. However, in looking around the internet and reading the book he gave me, it would appear that I am more in the low risk (wishful thinking?).

He gave me the litany of treatment choices telling my wife and me not to be concerned about keeping our plans to travel to Europe in September + October of this year.

So, I've been studying the various treatments. I think for now I might try Active Surveillance (AS) for awhile.

I would be interested in hearing opinions on my risk level and on the AS approach for others on this forum.

Please let me know if there is other information that I should have included.

Sept. 5, 2013 - Consulted with a radiology oncologist who looked at my biopsy results, and thought that Active Surveillance would be the correct approach for me. A week later my urologist agreed. He wanted me to check PSA twice a year.

Nov. 13, 2013 - I had a CT scan done on my abdomen and pelvis to diagnose a pain on the right side. Diagnosis revealed a right side inguinal hernia which I had repaired surgically on Dec 19.

** noted on the CT scan report was an enlargement of the prostate measuring 4.4 X 5.8 cm. -I wonder if this is a significant enlargement.

Dec. 11, 2013 ~ ~ PSA dropped from 8.2 to 5.1.

~~I wonder why the PSA jumped from 3.7 to 8.2 in a year, and then back down to 5.1 the following year. Is this an uncommon fluctuation? Was the 8.2 a false reading?

I am being monitored by my family GP. I am considering a change of doctors as he an his office are not as expeditious as I expect in setting up appointments, and notifying me of PSA results.
